GP CARE PLAN & CHRONIC DISEASE MANAGEMENT

Any patient with a chronic (long term) medical condition such as - diabetes, asthma, arthritis, cancer, heart disease or osteoporosis will benefit from a GP Care Plan and Chronic Disease Management.

GP Care Plan includes a team care arrangement, where the doctor determines which other health care provider or allied health professionals will the patient benefit from and involves them in providing treatment. With the patient’s consent, a relevant allied health professional is requested to be a part of the care plan. All GPMP and TCA plans are bulked billed by your GP, so there will be no charge for these services.
