Horizon Family Doctors is a mixed billing medical practice in Forest Lake. The team of medical practitioners at Horizon Family Doctors share a passion for providing the very best of care at all times. We focus on building relationships with our patients, to attend to immediate needs while collaborating on proactive strategies to achieve optimal health outcomes.
We believe in a holistic approach to help families thrive, by providing a range of medical services. We have a team of doctors who share a wide range of interests including women/men health, chronic disease management, skin cancer, geriatric care, and gynecology support.
Constantly developing our areas of particular interest – as well as general practice - we also work hard to maintain close working relationships with a wide range of specialists, providing referrals as required.
Being a part of a multicultural community, the majority of our doctors and nurses are multilingual: English, lgbo, Hindi, Punjabi, Farsi, Mandarin and Malayalam.
Our practice is well- located for the residents of Forest Lake, Richlands, Heathwood, Oxley, Inala, Durack, Ellen Grove, Carole Park and Darra.
We also provide a wide range of allied health services on-site including psychologist, dietitian, podiatrist, Australia Hearing, and pathology services.
Pharmacy, Physiotherapy – next door and Radiology services are also conveniently located nearby.
We know it can be difficult sometimes to remember appointments. Our patients are reminded to attend appointments by SMS or phone call.
A computerized reminder system is set in place for our patients, to remind them about upcoming/ overdue immunisation, blood test, health checks and other important health intervention.
However, Our Nurses are also determined to ensure that you do not miss out on any health intervention, thus they are responsible to actively follow- up on your health alerts via phone calls.
To monitor your health as well as fulfil our ethical & legal responsibilities, we require that you attend your regular doctor (or if your usual doctor is not available, one of our other doctors) to receive further prescriptions for your medications.
Whatever your preferred language, we can help you organise an interpreter to assist with any language barriers. We also arrange interpreters through NABS - National Auslan Interpreter Service, for our deaf patients.
Horizon Family Doctors uses the Telephone Interpreter Service 1300 131 450 when required. We can organise an “over-the-phone” service or with 48 hours’ notice an “on-site” service.
Your medical record is a confidential document. It is the policy of this medical centre to always maintain security of personal health information and to ensure that this information is only available to authorised members of staff.
